Minors

Submission of documents for minor citizens is possible only in the person of their lawful representative (one or both parents / the lawful tutor).

To submit documents for a minor citizen (irrespective of them holding their own passport), in addition to the documents required must be provided:

• a birth certificate of a minor (an original document and a copy / a verified copy by notary). If a parent or minor has changed his / her name after receiving the child’s birth certificate, he / she should provide documents verify it for example: marriage certificates / divorce certificates or certificate from Bureau of Acts of Civil Status (an original document and a copy / a verified copy by notary);

• a filled-in application form signed by both parents on 4th page of the application form along with a photo (photo requirements);

• a copy of the minor’s international passport (the one with his/her personal data and his / her photo);

• if the minor name is endorsed on either of the parent’s passport, to provide a copy of the first page of that passport and a copy of the page on which the name of the child is endorsed;

• applying for a single-entry visa for a child and if a minor is traveling with parents, the following documents should be provided:
- a confirmation of an accommodation shared with the minor;
- round-trip tickets for the parents and the child;
- financial guarantees by any parent;

At presence of valid visas of the parents:
- a copies of the valid visas, the first pages of the parent’s international passport with personal data and originals of their passports;

• if a minor is traveling with one of his parents, the following documents should be provided:
- a confirmation of an accommodation shared with the minor;
- round-trip tickets for the parent and the child;
- a copy of the valid visa, the first page of the parent’s international passport with his / her personal data and his / her original passport;
- a notarized consent form signed by the second parent (both original and a verified copy by notary);
- the parent who signs the consent form should provide a copy of his / her internal passport (pages with his / her photo and registration);
- financial guarantees by parent who traveling with a child;

• if a parent leaves the Shengen countries earlier than his child, or arrives there later, or leaves the Shengen countries for some time during one trip, he/she should provide:
- a notarized consent form signed by the parent (an original document and a verified copy by notary)
- a copy of the internal passport of the one who signs the exit permit (pages with his / her photo and registration);
- financial guarantees by any parent;

• if a child travels with an attendant, the following documents are required:
- a confirmation of an accommodation shared with the minor;
- round-trip tickets for the attendant and the child;
- a copy of the valid visa, the first page of the attendant’s international passport with his / her personal data and his / her original passport;
- a notarized consent form / letter signed by both parents (original documents and a verified copy by notary);
- copies of internal passports of both parents (pages with their photos and registrations);
- financial guarantees by any parent;

If a parents giving a notarized consent form/ letter to a grandmother or grandfather for accompanying a child should provide documents confirming relationship with a grandchild.

• if a minors are travelling without any attendant (only if the child is already 14 or older), the following documents are required:
- notarized consent forms / letter signed by both parents (both original documents and a verified copy by notary);
- copies of internal passports of both parents (pages with their photos and registrations);
- financial guarantees by any parent;

• if a child travels with his/her legal guardian, the following documents are required:
- a confirmation of an accommodation shared with the minor;
- round-trip tickets for the legal guardian and the child;
- a copy of the valid visa, the first page of the guardian’s international passport with his / her personal data + his / her original passport;
- documents proving the termination of parental rights or the loss of parents (original documents and a verified copy by notary);
- court decision on the appointment of a guardian (original documents and a verified copy by notary);
- consent form from the legal guardian (original documents and a verified copy by notary);
- financial guarantees by legal guardian;

if a child travels with an attendant, the following documents are required:
- a confirmation of an accommodation for the trip shared with the minor;
– round-trip tickets for the attendant and the child;
– a copy of the valid visa, the bio-page of the attendant’s international passport and original passport;
– a notarized consent form
, issued no more than one year ago, signed by both parents (original documents and copies / a verified copy by notary);
– copies of internal passports of both parents (pages with their photos and registrations);
– financial guarantees by any parent;

• applying for a longtime visa for a child, the following documents are required:
- one provides two reciprocal notarized exit permits signed by both parents (both mother and father provide a reciprocal exit permit) for the whole period of the applied visa. Original documents and a verified copy by notary;
- copies of internal passports of both parents (pages with their photos and registrations);

• in case of deceased parent an original death certificate or in case of a certificate of termination of parental rights are provided under the following circumstances (original documents and a verified copy by notary);

• form № 25 (a certificate taken in a regional Register Office) is required, when the father’s name is mentioned by the mother choice on child’s birth certificate (original documents and a verified copy by notary);

• if a parent has gone missing or his whereabouts is not known, it is necessary to provide an court decision (original documents and a verified copy by notary);

• if in the birth certificate in the column “father” there is a crossed out section, it is necessary to provide the original document and a verified copy by notary.
